Medical Clearance Checklist for New and Transfer Varsity Student-Athletes

Updated Pre-participation exam (Physical): upload your physical into your Teamworks account. Please have your medical provider complete the Preparticipation Exam (PPE) form, in addition to any other forms they may use. Additional documentation is required if you are being treated for certain medical conditions. (i.e. cardiac). New and Transfer Student Athletes: must be within 6 months prior to the start of their first day of regulated participation, per NCAA regulations.

Read and Sign NCAA Sickle Cell Trait Fact Sheet

Upload Sickle Cell Trait test results into your Teamworks account Note: One time only for New and Transfer students. The NCAA requires specific documentation regarding testing and treatment for the sickle cell trait and/or sickle cell anemia. According to the 2022 NCAA legislation, student-athletes no longer have the option to sign a waiver declining confirmation of their sickle cell trait status. Student-athletes must upload sickle-cell trait testing results clearly indicating sickle cell trait status. This health record will state the date of testing and results. We encourage you to check with your treating physician regarding birth records prior to requesting a new test. This information is necessary to be able to compete in intercollegiate athletics and must be on file prior to participation.
