Medical Clearance Checklist for Returning Varsity Student-Athletes

Log into your Teamworks account and complete the following required forms.

Updated Pre-participation exam (Physical): upload your physical into your Teamworks account. Please have your medical provider complete the Preparticipation Exam (PPE) form, in addition to any other forms they may use. Additional documentation is required if you are being treated for certain medical conditions. (i.e. cardiac).

ADD / ADHD Medication Exemption Waiver (If applicable): Completed by your physician, upload to your Teamworks account.

Asthma Medication Exemption Waiver (If applicable): Completed by your physician, upload to your Teamworks account.

Upload a copy of the front and back of your insurance card to your Teamworks account.
